Recent studies have demonstrated that ischemic tolerance in the central nervous system could be induced in humans as well as in small animals. The mechanism other than the synthesis of neuroprotective proteins may play a pivotal role. The advances in DNA microarray technology have revealed changes in gene expression induced by preconditioning. It has been reported that ischemic preconditioning may reprogram the response to ischemic injury that resembles hibernation. Preconditioning with anesthetics (isoflurane and morphine), hyperbaric oxygen, and ATP sensitive potassium channel openers seems to have potentials and may be applicable in the clinical setting.<BR>Ischemic tolerance may enable us to use the new strategy for the brain and spinal cord protection in the near future.
