The longitudinal study of physical status and persistent infection in women with HPV16

Integration of human papillomavirus (HPV) DNA has been considered a late event in cervical carcinogenesis. However, the integrated form of HPV is frequently detected in a normal cytologic background of the uterine cervix, and it has been unclear whether this is associated with the development or progression of cervical intraepithelial neoplasia (CIN). The present longitudinal study using quantitative real-time PCR was conducted to evaluate the significance of detecting the physical status of HPV16 (episomal form), integrated form, or mixed episomal and integrated form) in cervical swabs obtained by self-sampling at short intervals from 11 women (3 with persistent and 8 with transient infection) with HPV16 among 19 women prospectively followed up for more than three years. The mixed episomal and integrated form wasdetected in 3 women with persistent infection, but undetectable in 8 women with transient infection, whereas only the pure episomal form was detected in women with transient infection and with resolution of persistent infection. The physical status of HPV16 was significantly associated with persistent or transient infection. The presence of integrated HPV16 is predictive of HPV16 persistence, and not progression of CIN, in women with normal cytology. Our findings also show that the presence of pure episomal HPV16 is important for predicting both HPV16 resolution and dynamics.
