Membrane Permeability and Receptor:With Relation to Leakage of Muscle Enzymes to Serum

The activities of creatine phosphokinase (CPK), aldolase and lactate dehydrogenase (LDH) in sera have been used as a indicator for the diagnosis of skeletal muscle diseases and heart muscle diseases. On the other hand, recent reports show that the MM type of CPK isozyme in sera can also increase in some muscle independent patients with such neurologic and psychiatric disorders as cerebro-vascular disease 2) and acute psychosis 3). So, a series of studies were made to elucidate the leakage mechanism of muscle enzymes.<BR>Using male rabbit 3kg weighing, medial (sympathetic) hypothalamus was electrically stimulated. The result was that the MM type of CPK isozyme, the A type of aldolase isozyme and the M<SUB>4</SUB> type of LDH isozyme increased insera 1). In addition, a subcutaneous injection of alpha (not beta) stimulator followed a increase of the MM type of CPK in sera, in association with a rise of Na-K ATPase activities of white muscle sarcolemma.<BR>These increasing on serum CPK activities by alpha stimulator were dependent on sex hormone. By a subcutaneous injection of alpha stimulator, some female rabbits showed the increase of CPK activities in sera, but others not. One or two weeks after a intramuscular injection of estrogen, none of male rabbits showed the same increase by alpha stimulator.<BR>Beta blockers were orally administered to Duchenne typepatients (male) with high activities of serum CPK and to their mothers (female) with or without a little elevated activities of serum CPK. After one or two weeks, there ocurred no appreciable difference on serum activities in patients, but a slight increase in all their mothers. When both ephedrine (having alpha and beta stimulating effect) and estrogen were orally administered to the patients, their serum CPK activities gradually decreased. But then, after discontinuation of both drugs, they rose again.<BR>These findings indicate that membrane permeability depend on the irritability of alpha/beta receptor, and that the irritating alpha receptor on male individuals induce muscle enzymes, especially white muscle enzymes, to leak out to sera, and also that the Duchenne type may have some alpha predominant disposition.
