Selection of Prosthetic Heart Valves

Between 1964 and 1981, 380 valve replacements were carried out in our institute. Of the 380, 285 operative survivors, which consisted of 166 isolated MVR, 88 isolated AVR and 31 A+MVR were analyzed by means of actuarial method. Hemodynamic studies were also carried out during late follow up period. In MVR, survival and event free proportins were significantly low in Kay-Shiley (KS) valves. Poor results were also obtained in Starr-Edwards (SE) 6500 valves. Severe thromboembolic complication and calcification were noted each in one case among 30 cases with xenograft valves. In AVR, there were slight differences in actuarial survival curves between several types of prosthesis. But event free proportions were significantly low in SE 2310/2320 valves, because of severe hemolysis and intermittently stuck ball. In hemodynamic study, Björk-Shiley (BS) valve showed excellent results both in MVR and AVR.It is concluded from this study, that BS valve would be a prosthesis of choice in both mitral and aortic positions.
