Accuracy of Left Atrial Imaging Using NavX Velocity System: Initial Experience and Validation

<B>Background:</B> Three-dimensional navigation is increasingly used during ablation of atrial fibrillation (AF). The NavX velocity system (NavX) aims to mold the virtual anatomy to the computed tomography (CT) image. However, the accuracy and clinical usefulness have not been elucidated. This study attempted to compare the left atrial (LA) CT and NavX geometry to assess the accuracy of the mapping systems. <B>Methods:</B> This study consisted of 15 consecutive patients undergoing AF ablation using the NavX. LA-NavX geometries were constructed with a circular catheters during AF, and compared to the corresponding CT. The diameter of each pulmonary vein (PV) and length of the roof and bottom lines were calculated with both the CT and NavX. The procedural duration to map LA was recorded to assess the clinical usefulness. <B>Results:</B> A total of 60 PVs were delineated in detail. The difference in the diameter of each PV between NavX and CT geometries were 2%, 1%, 1%, and 15%, for left-superior, left-inferior, right-superior and right-inferior PV. There were significantly differences in the roof (50.8±5.9 vs. 43.8±5.1 mm; p<0.01) and bottom lines (63.9±7.6 vs. 53.2±8.9 mm; p<0.01) between NavX and CT. A significant (P<0.05) inverse curvilinear relationship existed between the case number and time required for imaging (r=0.55). <B>Conclusion:</B> Image delineation of the LA using NavX is relatively accurate, while horizontal elongation is documented, and associated with a progressive reduction in the procedural duration.
