We report on a 73-year-old man who had been diagnosed with dilated cardiomyopathy by myocardial biopsy 6 years before. He has been also suffered from renal failure on HD for 3 years. In spite of appropriated medications, previous echocardiography showed very poor left ventricular contraction (EF<10%) with marked chamber dilatation and he had gradually worsened the severity of heart failure and forced to admit repeatedly. He has been suffered from atrial fibrillation sustained more than 6 years. He was taking amiodarone, carvedilol, and his average resting heart rate was 59 beats per minute before ablation. These results suggested that the LV dysfunction was not due to tachycardia induced cardiomyopathy. We performed catheter ablation of chronic atrial fibrillation (EEPVI+LA roof+CTI) and could maintain sinus rhythm. After ablation, left ventricular function improved gradually, and recently echocardiography showed normal left ventricular contraction at 10 months after ablation. BNP decreased from 885 to 46 pg/mL. We report the patient whose LV function dramatically improved after ablation of atrial fibrillation.
