A case report of pigmented villonodular synovitis in early stage.

Many cases of pigmented villonodular synovitis (P. V. S.) have been reported. But P. V. S. is not likely to be diagnosed in early stage. We experienced a case of P. V. S. of the early stage with a plica synovialis mediopatellaris (shelf).The patient was a 16-year-old girl, who had suffered from arthralgia and effusion (not bloody) with occasional locking and giving way in the left knee joint for two years.Radiogram showed normal findings. Arthrogram revealed a shadow of shelf plica in the medial side of the knee. Through the arthroscopy an incomplete shelf and an abnormal synovial mass were found in the medial compartment. We performed surgical excision of the shelf and the mass (6cm×2cm), which included some nodules. The diagnosis of P. V. S. was made at surgery and was confirmed histologically.The patient had an uneventful postoperative course and was free of symptoms eight months after surgery.
