We experienced a rare case with fracture of the outer clavicle displaced backward to the glenoid cavity of the scapula in a five-year-old boy.He was injured by traffic accident and had multiple injuries. Operation was carried out ten days after injury. it was revealed that the acromioclavicular ligament was intact and that the proximal fragment displaced backward to the glenoid cavity of the scapula leaving the periosteal sleeve. Open reduction and internal fixation with K-wire was done.Clinical result was favourable. The fracture mechanism was suggested.
