Pubic fractures in old age.

In old age, sustained fractures are numerous after a minor injury. We examined five women patients, aged 69-86 (mean, 77.4), with pubic fractures. Their subjective complaints were groin pain and difficulty in walking and objective findings were limitation of range of motion of the hip joint and tenderness around the origin of the adductor muscles. Although the fractures were not clear in plain radiographs, scintigraphy revealed an intense increase in radionuclide uptake, and therefore the fractures were diagnosed by clinical findings and radionuclide bone scan findings.We considered osteoporosis as a causal factor of the fractures and evaluated the degree of osteoporosis by means of the trabecular pattern and MD method. We treated the fractures conservatively by resting patients during acute periods and administered Ca, active vitamin D3 and calcitonin. To prevent the osteoporosis from worsening due to immobility, we allowed the patients to walk as early as possible.From the results obtained with these patients, conclude that clinical findings and radionuclide bone scan are useful for diagnosing pubic fractures.
