It is mostly acceptable that MR Imaging is more accurate, sensitive and specific than conventional diagnostic methods of vertebral spondylitis (VS).We have studied three cases of VS and one case of vertebral tuberculosis (VT) by comparing MRI with their pathology. In conclusion, first, acute inflammation shows high signal intensity, resulting from the production of an exudate containing polymorphonuclear leukocytes and fibrin. Second, healing of the inflammation shows low and/or iso-intensity in consequence of fibrosis and osteosclerosis.In addition we investigated the difference in MRI findings between VS and VT. Changes of VS begin at vertebral end-plate and advance to narrowing the of disc space and epidural abscess. On the other hand VT shows destruction of the vertebral body, sometimes involving posterior element, and anterior-subligamentous abscess without narrowing of the disc space.
