Serum bile acid composition in patients before and after percutaneous transhepatic cholangiodrainage for obstructive jaundice.

Serum bile acid composition was investigated in 11 patients undergoing percutaneous transhepatic cholangiodrainage (PTCD) for obstructive jaundice and 30 healthy subjects. Free, glycine-conjugated, taurine-conjugated and sulfated bile acids were separated using PHP-LH-20. Each bile acid was further fractionated into lithocholic acid (LCA), deoxycholic acid (DCA), chenodeoxychloic acid (CDCA), cholic acid (CA) and ursodeoxycholic acid (UDCA) using gas liquid chromatography.Free LCA, CDCA, CA and UDCA and glycine-conjugated CDCA tended to be higher in the healthy subjects aged between 51 and 66 years than in those aged between 25 and 50 years.The following findings were obtained from the patinents before PTCD: (1) Total serum bile acid was higher than the normal value. (2) Taurine-conjugated, glycine-conjugated, sulfated and free bile acids were composed 46.6%, 44.2%, 8.6% and 0.6% of total serum bile acid, respectively. (3) Primary bile acids, such as CDCA and CA, were composed 97% of total serum bile acid.The findings obtained after PTCD were as follows: (1) Total serum bile acid decreased by 26.6% after 3 hours and further by 93% after 120 hours, approximate to the normal value. (2) No change was noted in free bile acid. (3) Glycine conjugate/taurine conjugate ratio increased, approximate to the normal value. (4) Sulfated bile acid decreased by 93% after 120 hours, but was still higher than the normal value. (5) CA/CDCA ratio also decreased, but was still higher than the normal value.
