Follow-up study on colonic adenomas.

In order to investigate the natural history of the colonic adenoma, 57 colonic adenomas in 31 patients were followed up by barium enema and colonoscopic examination for mean duration of 36 months. The histology of these adenomas was examined on the specimens obtained by biopsy or polypectomy, and their histologic atypia was graded according to the Morson's criteria. The adenomas beyond 5.1mm in initial size of maximal diameter were found to be enlarged more markedly than those under 5.0mm in size, with statistical significance(p<0.05). As regards to the relation of the location to the changes in size of the adenomas, the right sided colonic ones tended to be enlarged more markedly than those in the left sided ones during the follow-up periods. Especially the adenomas in the ascending colon were enlarged much more than those in the rectum, with statistical significance(p<0.05). The increase of the size of the colonic adenomas was found to be proportional to the grade of the atypia on final histological findings of the specimens(p<0.005).These results indicate that the size and location of the colonic adenoma, and also the grade of the histological atypia of the colonic adenoma are postulated to be the essential factors for their evolutional history.
