The distant transfer of the free skin flap by microvascular anastomosis opens a new era in reconstructive plastic surgery and herein the author's personal experiences regarding to the microvascular surgery are described in details in the following subjects; introduction, concept of microsurgery, microvascular instruments, fundamental techniques of microvascular anastomosis, experimental study in dogs, one staged transfer of free skin flap in clinical cases, and advantages and disadvantages of microvascular transfer of skin flap.
