Clinicopathologic Studies on Neural Cell Adhesion Molecule of Rectal Cancer with Special Reference to the Perineural Invasion.

Two hundred seventy-seven cases of rectal cancer were studied immunohistchemically to assess clinical and pathological significance of neural cell adhesion molecule (NCAM) in rectal cancer. Relationship between NCAM and various clinicopathological factors also were examined. (1) NCAM was positive in 43 cases, at a rate of 15.5%. (2) Of clinicopathological factors which determined prognosis, there was statistically significant difference between NCAM and the following factors ; perineural invasion, venous invasion, lymphatic permeation, liver metastasis, depth of invasion, histologic type, lymph node metastasis and inflammatory cell infiltration. In multivariate analysis, perineural invasion had the strongest relationship to NCAM. (3) In evaluation of recurrence or survival rate of the radical operation, Dukes C cancer with positive NCAM seemed to have a high local recurrence rate and had a significantly low 10-year survival rate than that of negative NCAM. The results suggested that NCAM not only is an important factor in determining prognosis of the cancer but also can be a tumor marker for perineural invasion.
