A treated case of typhoid fever with ileal perforation.

We treated a 17-year-old girl who had typhoid fever with ileal perforation. She came to the hospital with chief complaints of pyrexia and eruption. Relative bradycardia and splenomegaly were observed, and abdominalgia appeared during clinical investigation. There was muscular defense over the entire abdomen, and X-ray examination revealed free gas under the diaphragm. The patient underwent emergency surgery under a diagnosis of perforated pertonitis. Since diffuse peritonitis due to ileal perforation was detected as a result of laparotomy, ileectomy, peritoneal perfusion and drainage were carried out. Macroscopic and histological examination of the resected specimen led to a diagnosis of ileal perforation due to typhoid fever. The postoperative course was uneventful, with administration of chioramphenicol. Perforation may occur unless typhoid fever is found in time. Pyrexia, accompanied by abdominalgia, is one of the conditions that is considered to be associated with typhoid fever, a nearly forgotten condition.
