興味ある十二指腸病変を合併した潰瘍性大腸炎の1症例

A 32 year-old female was admitted to our hospital complaining of epigastralgia and soft frequent stool for 3 months. Laboratory datas showed as follows : WBC 9600, Hb 8.2g/dl, Cholesterol 111mg/dl, CRP 2+, ESR 24mm/hr. PPD skin test was positive. Faces culture of acid-fast bacillus was negative. Barium enema study showed disappearance of haustral marking, rough mucosal pattern and narrowing through all of the large intestine. By clinical and histological findings of this colonic lesion was diagnosed as the ulcerative colitis. Furthermore, roentgenological examination of the duodenum revealed disappeared mucosal fold, shortning and narrowing from bulbus to fourth portion of the duodenum. Endoscopic examination revealed multiple shallow ulcerations in the duodenum diffusely. The histological findings showed cryptitis and crypt abscess with diffuse inflammatory chaniges in the duodenal mucosa. It was concluded that the duodenal lesions had a similar genesis to the ulcerative colitis.
