I. Sigmoid Colon Volvulus and Ulcer Formation

Though there, are many varieties of ulcerative disease in colon, little has been said about ulcer formation in sigmoid volvulus. From 1971 to 1976, total of 103 cases of mechanical bowel obstruction were met with in our hospital, sigmoid volvulus comprising 9 of those cases. Ulcer formation was found in two cases in resected specimen or by ' colonofiberscope. Both of them were longitudinal along the long axis. The former consisted of multiple linear ulcers at mesenteric side and the latter was solitary and 10 cm in length. Pathologically there were submucosal edema and cell infiltration and additionally, in the former, there was marked venous congestion.The vascular changes have been accused as one of the factors causing colitis or colonic ulcer but it has been little clarified as whether primary lesion is on arterial side or venous side. The various pictures of colon in sigmoid volvulus can be attributed to the disturvance of venous return. In conclusion. the venous congestion could be the cause of the colonic ulcer formation on mesenteric side in sigmoid volvulus and this could be attributed to the colonic pathological condition.
