術前MRIが有用であったfibrolamellar hepatocellular carcinomaの1例

The patient is a 29 year-old man with fibrolamellar hepatocellular carcinoma (F-HCC). He was given a diagnosis of focal nodular hyperplasia at another hospital in August 2005. He suffered from abdominal pain, and he was referred to our hospital for further examination. Computed tomography showed a hypervascular tumor, 10 cm in diameter in the right lobe of the liver. A hyperintense tumor with a hypointense central scar within the tumor was seen on T2-weighted magnetic resonance images (MRI). A hyperintense tumor was seen on super paramagnetic iron oxide particle MRI. No tumor biopsy was performed. He was given a diagnosis of F-HCC, and right lobectomy was performed. The cut surface of the resected specimen showed a firm, lobular tumor, measuring 10 cm in diameter with a fibrous central scar. Microscopically, the tumor was typical F-HCC which consisted of eosinophilic tumor cells and fibrous connective tissue of lamellar pattern. In conclusion, we encountered a case of F-HCC in a patient who was given a diagnosis preoperatively with imaging modalities.
