糖尿病における下垂体TSH分泌動態に関する臨床的研究

TRH test with 100μg of synthetic thyrotropin releasing hormone (TRH) was performed in 37 patients with diabetes mellitus. Although peak concentration of plasma TSH in response to TRH in diabetes mellitus (18.9±2.5 μU/ml) was slightly but non-significantly higher than that in control subjects (13.1±1.7 μU/ml ; n=10), they were classified as subnormal (Group 1), normal (Group 2) and supranormal (Group 3) according to the the peak concentration of TSH response. Ten out of 37 patients with diabetes mellitus showed subnormal TSH response and 10 showed supranormal response. Thus, 20 patients out of 37 (54%) showed an abnormal TSH response to TRH. Duration of diabetes mellitus and fasting blood glucose concentrations were Group 3 > Group 2 > Group 1. These findings suggest that abnormal TSH response to TRH may result from abnormal carbohydrate metabolism. Moreover, high incidence of diabetic complications was observed in Group 3 (50%) as compared with Group 1(17.6%). This finding may indicate that diabetic complications induced by disturbances of carbohydrate metabolism have an important role on the abnormal TSH secretion. The present investigation has demonstrated that abnormal pituitary TSH secretory function is frequently found in patients with diabetes mellitus and that abnormal TSH response to TRH in patienrs with diabetes mellitus seems to correlate with the disorders of carbohydrate metabolism or the presence of diabetic complications.
