Clinical survey was made of 42 cases of Sialolithiasis of the submaxillary gland that were cured of the disease by surgical removal of calculi in our clinic during the period from July, 1950 to June, 1959. An attempt was made to evaluate two different surgical procedures, especially to establish criteria for selection of either one, from the point of view of sialography. Diagnosis of the cases was made based on the location of calculi at the time of their surgical removal, and they were divided into the following three groups : cases with calculi in the Wharton's duct, in the transitional part and in the gland itself. Based on sialograms, the following criteria were established to select either intraoral or extraoral procedure of surgical treatment for lithiasis of the submaxillary gland : 1. Calculi in the gland itself are to be removed together with the gland and Wharton's duct (extraoral procedure). 2. As a principle, calculi in the Wharton's duct and transitional part are to be removed by intraoral procedure, but extraoral procedure is preferable in cases, in which there are multiple calculi, hypofunction of the gland due to senility is feared, sialograms show leak of contrast medium and palpation show abscess formation, there are severe inflammatory changes or there have often been recurrences of the disease. Remote effects of the surgical treatment have confirmed the validity of the criteria set up by us for this disease.
