Pancreatic Abnormalities in Multiple Endocrine Neoplasia Type I : Immunohistochemical and Morphometric Studies

Pancreatic tissues from 4 cases of multiple endocrine neoplasia type I (MEN I) were stained for insulin, glucagon, somatostatin, pancreatic polypeptide (PP), gastrin, vasoactive intestinal polypeptide (VIP), and neuron specific enolase (NSE, or γ-subunit of enolase) by the immunoperoxidase method, and analyzed histomorphometrically. All four pancreases included PP-poor lobe and three pancreases included PP-rich lobe (dorsal part of the pancreatic head) as well. All cases had multiple adenomas and total number of 87 adenomas were found. The type of the adenomas with greatest number was glucagonoma. And the number of adenomas containing four pancreatic hormones was the greatest. There was neither increase in total area of endocrine cells nor increase in islet size in all pancreases. The findings strongly suggest that the origin of multiple adenomas of the pancreases in MEN I is not mature islets. The number of small clusters of endocrine cells with a certain pancreatic hormone was greater than that of normal controls. They might represent progenitors of adenomas or reactions to hormonal abnormalities.
