Refusing blood transfusion by Jehovah's Witness patients causes not only ethical and legal problem, but also subjects for medical decision of practice in anesthesiology. The authors experienced 11 patients who opposed blood transfusion because of their believes. According to our hospital guideline to those patients, we attempted to accept blood and its product if they were indicated. There were a few patients decided to accept blood whenever it was medically indicated. There are individual problems regarding their final decision for the informed consent. We introduced hypotensive technique with hemodilution technique for three patients who were expected massive blood loss during surgery. Arterial oxygen content and cardiac output are the most important indices during procedure.
