Traumatic Epilepsy due to Work-related Accidents

The incidence of traumatic epilepsy was studied in 387 patients who had been injured by industrial accidents on duty or traffic accident on the ways to or from their official working places. These patients were interviewed by the author, and clinical data including the case history, CT films, X-ray films and EEC records were reviewed. All cases were classified by Araki's method. As a result, the types I and II had only 2 cases of epilepsy in 232 patients (0.86%), and the types III and IV had 14 in 155 (9.03%). Relationship between the severity of trauma and the high incidence of traumatic epilepsy was proved statistically (P<0.001.) Eight cases in the severely injured group had mental deficit, but the lightly injured group (head injury type II) showed no residual mental impaiment. EEC records could be satisfactorily checked on 12 cases and 9 patients showed abnormal EEC (75%). High incidence of epilepsy was found in the groups of the acute subdural and the intracerebral hematomas, as well as in that of the focal contusion. After the intracranial hematoma or the focal contusion, iron from hemorrhagic site seemed to be responsible to the formation of epileptic focus. As an outcome, 8 cases in 16 (50%) were free from attack by successful medication.
