Superficial Bladder Cancer With a Massive Hematoma After Perineal Blunt Injury : Report of a Case

A rare case of superficial bladder cancer with a massive hemation after perineal blunt injury was reported. A 66-year-old male complained of lower abdominal pain and urinary retention about twelve hours after the injury. Following excretion of a large amount (1,060 ml) of hematuria through a transurethral catheter, hematuria became gradually bloody and the excretion frequently stopped in spite of the catheterization. Therefore, catheter vesicotomy was necessary for urine excretion, and 2,400 ml of the blood was transfused for the maintenance of vital signs during 8 days up to the surgical therapy. Morphologic examinations revealed a protrusive tumor into a massive hematoma in the bladder. At the time of operation, however, five tumors were found, and the bleeding point was certified to be the stalk of the biggest one. After removal of the tumors and 490 g of hematoma, his urination was uneventful. Microscopically, all the tumors were superficial transitional cell carcinomas. The pathogenesis concerning the acute onest of urinary retention with a massive hematoma is discussed.
