Dynamics of Hyaluronate in Patients with Pheumatoid Arthritis(Special Issue on "Hyaluronan")

Hyaluronate(HA)levels were much higher in rheumatoid arthritis(RA)patients, compared with the values of normal controls measured by the aid of HA assay using hyaluronic acid binding protein(HABP). It is, however, surprising that all RA patients in the severe joint destructive stage did not always have high plasma HA level. No clear correlation between HA level and clinical laboratory data such as blood sedimentation ratio(BSR)was found, while CRP titer was the only laboratory finding to be correlated with plasma HA levels though significance was not high. The cases with joint destruction in large joints within a year revealed high HA levels compared with those cases without active destruction. Furthermore, in a four-year follow-up study, one case with high plasma HA levels showed joint destruction of the knee from grade IV to V within a year. From one-year comparative and long year follow-up studies, plasma HA levels do not reflect the destructed state but rather the progressive deterioration of the joint.
