Plasmapheresis for the Treatment of Macroglobulinemia

We report here the lgM removal rate in a patient by applying various methods of apheresis for the treatment of macroglobulinemia. The patient was a 64-year-old man. For the treatment of hyperviscosity syndrome, apheresis was performed in combination with chemotherapy. Simple plasma exchange (PE) was performed twice and double-filtration plasmapheresis (DFPP) with 5% albumin used as replacement fluid was performed 21 times; 17 times when the IgM level in the blood was 6,000 mg/dl or more. and 4 times when lgM level was less than 6,000 mg/dl. Post-centrifugal plasmapheresis (PCPP) was performed 8 times, and DFPP with physiological saline solution as replacement fluid was performed 9 times. The IgM removal rate from the blood was 42% in PE, and in 5% albumin-DFPP, it was 18% when the IgM level was 6,000 mg/dl or more and 36% when less than 6,000 mg/dl. When PCPP was used, the removal rate was 45%, and with saline-DFPP, the rate was 47%. When the IgM level was 6,000 mg/dl or more. PCPP was the most efficient method, and when IgM was less than 6,000 mg/dl, saline-DFPP was sufficient.
