Efficacy of hochuekkito for postprandial hypotension complicated by diabetes

• 症例は72歳, 男性。1970年より糖尿病にて加療中。1994年より食後に短時間の意識消失発作を自覚するも放置していた。2004年に入り発作回数が増加したため, 精査加療目的で入院。臥位で施行した75g oral glucose tolerance test (OGTT)で, 眠気を伴った26mmHgの血圧低下を認め, 食事性低血圧と診断。また, 血中noradrenaline (NA)の基礎値および反応性の上昇(前476→30分後561→60分後689pg/ml)を認めた。食後の眠気に対し補中益気湯投与開始。1週間後の75g OGTTで血圧低下は12mmHgと改善し, 眠気は消失した。また, 血中NAの基礎値および反応性の変化(前242→30分後270→60分後254pg/ml)を認めた。補中益気湯は, 75g OGTTでの血圧低下を改善し, 食後の眠気を改善した。その機序に末梢交感神経機能に関与している可能性が示唆された。
