Survival Probability, Delayed Mortality, and Relationship Between Short-term Outcome and Long-term Survival After Aneurysmal Subarachnoid Hemorrhage

くも膜下出血後の長期生存率・遠隔期の死亡原因・短期予後と長期生存との関係について調べるため、106例について5年以上の後見的追跡調査を行った。6ヶ月時に経過良好だった75例中、8例がその後死亡していた。一方、6ヶ月時severely disable，vegetativeだった9例中7例は、5年以内に死亡していた。その7例中5例は、来院時に脳室内出血を合併していた。6ヶ月以降に死亡した計15例中5例は、脳室-腹腔シャントに関係した合併症で死亡していた。結論として、6ヶ月後の予後は長期生存ならびに遠隔期死亡の重要な予測因子である。短期予後を改善するため脳室内出血の管理をさらに発展させ、遠隔期死亡を減らすためシャント術施行患者を注意深く観察しなければならない。
