Apneic Spells in a Patient with Myelomeningocele without Chiari Type II Malformation : Case Report

無呼吸発作を繰り返すChiari II型奇形非合併脊髄髄膜瘤(HMC)患児の1例を報告した。症例は新生児女児。腰仙部の開放性MMCに対し、出生日に修復術を施行。生後2日目より無呼吸発作が出現し、レスピレーターの装着を要した。生下時認められたABRの異常所見はその後徐々に増悪した。シャント術後も無呼吸発作、ABR所見ともに改善をみず2歳で死亡した。これまでにChiari II型奇形非合併例における脳幹障害の報告はない。本症例は、Chiari奇形の有無にかかわらずMMCの脳幹に固有に備わっているintrinsic dysfunctionとでもいうべき機能障害の存在を示唆するものである。同機能障害を文献的に考察するとともに、MMCにおける呼吸不全についてreviewして報告した。
