Neuroimaging Findings of the Development and Resolution of Solitary Brainstem Abscess: Characteristics of Neuroimagings in the Early Stage of Brainstem Abscess and Importance of Surgical Management for Brainstem Abscess : Case Report

64歳女性で孤立性の脳幹部膿瘍を認めた症例を報告した。初発症状は複視と頭痛で、神経所見は右動眼神経麻痺および両眼の左向き眼振であった。発症時のCT及びMRIは異常を認めず、血液検査上も炎症所見は見られなかったため、虚血性疾患として治療を受け、1カ月後に症状は消失した。その後、神経症状の再燃及び髄膜炎様の所見等が見られ、4カ月後の画像検査で脳幹部に膿瘍の形成が見られた。第4脳室経由で膿瘍の穿刺排膿ドレナージを施行し、術後のCT及びMRIで膿瘍は消失した。本例ではMRIおよびCT脳幹部膿瘍の形成と改善を経時的に観察することが出来た。画像検査による経過観察が、治療方法の決定に重要であった。
