Neurovascular Decompression for Trigeminal Neuralgia in Elderly Patients

三叉神経痛に対する神経血管減圧術の成績を75歳以上の患者(elderly group-17人)と75歳未満の患者(nonelderly group-115人)の二つのグループで比較した。elderly groupでは術前にカルバマゼピンで治療した症例が多かったが、二つのグループ間には手術成績に統計学的な差はなかった。神経血管減圧術は高齢者の三叉神経痛でも良好な成績をあげられることが確認できた。カルバマゼビン等の治療が無効になった場合には神経血管減圧術を考慮するべきである。但し80歳を越える場合には手術は特に慎重を要する。superior petrosal veinやその他の静脈の温存を図り、小脳のretraction等は最小限に止めるべきである。
