Hemangioblastoma Mimicking Tentorial Meningioma : Preoperative Embolization of the Meningeal Arterial Blood Supply : Case Report

72歳男性で、テント下面に付着部をもち小脳半球内に埋没していた血管芽腫の症例を報告した。本例では腫瘍の栄養血管は椎骨動脈系以外に外頚動脈の分枝である後頭動脈の硬膜枝、中硬膜動脈の後枝、踈孔よりのテント枝、および内頚動脈よりの硬膜下垂体動脈の分枝であるテント動脈であった。術前外頚動脈よりの栄養血管に塞栓術を行い、術中出血を最小にすることが出来た。後頭蓋窩血管芽腫の20%が硬膜に付着を持つが、その大多数が再発で、本例の様に初発例では稀である。テントおよび硬膜に付着する腫瘍の鑑別として重要で、術前塞栓術は有効である。
