高血圧症をもつ72才男性が、右被核を中心として脳内出血をきたした。血管撮影で、シルビウス裂の径4cmの脳動静脈奇形が存在した。手術により奇形および血腫を全摘した。高齢者の脳動静脈奇形の出血はまれであるが、高血圧性脳出血の鑑別診断上忘れてはならない。過去の報告をみると、65才以上の高齢者で血管撮影上明確な脳動静脈奇形による出血をまとめたものは非常に少ない。それらの文献を狩猟し、考察を加えた。本例は少なくとも文献上、該当する最も高齢の患者であった。
