Anomalous Internal Carotid Artery-Anterior Cerebral Artery Anastomosis Associated with Fenestration and Cerebral Aneurysm : Case Report

稀な血管奇形であるICA-ACA anastomosisの一剖検例を報告した。症例は69歳女性、SAHにて発症し当院搬送入院。右内頚動脈撮影にてIC, cavernoos portion直上より分岐し前内方へ向かい、正中で上方へ方向を転じる奇異な走行をする異常血管を認めた。異常血管はdistalでfenestrationを形成し、その遠位部に動脈瘤を認めた。右側のA1は造影されなかった。発症3日目、急激な肺鬱血により死亡した。剖検上、動脈瘤とfenestrationを伴うinfraoptic courseをとる異常血管が確認された。また、異常血管とdistal ICから視神経を養う無数の穿通枝が認められ、本奇形がprechiasmal arterial plexusのvariationであるとの仮説を支持するものと考えられた。
