Hemangiopericytoma Arising at the Site of a Dural Graft 13 Years After Total Removal of a Benign Meningioma : Case Report

• 64歳の女性が進行性に拡大する左側頭部の皮下腫瘤を主訴として当院を受診した。同症例は13年前に左蝶形骨縁髄膜種の全摘術を受けていたが2年前までに髄膜腫の再発は認められていなかった。画像検査上、皮下腫瘤は硬膜に接着し前回手術による頭蓋骨欠損部を通して頭蓋外に発育しており、また肺に多発性の腫瘍を認めた。髄膜腫の再発及び肺転移と診断し腫瘍切除術を行ったところ、腫瘍は前回手術時に硬膜欠損部に移植された筋膜の断端から発生している所見があり、組織診断は血管外皮腫であった。本症例では同腫瘍がいわゆる瘢痕腫瘍として発生した可能性が考えられた。良性腫瘍全摘術後でも長期追跡が必要であることを示す症例と考えられた。
