• 重度の脊髄症状を認めた環椎横靭帯骨化症の1例を報告する.症例は68歳, 男性.56歳時, 頚髄損傷にて前方減圧及び固定術後, 進行性四肢筋力低下著明となり当科入院となった.画像所見より歯突起周囲の靭帯は石灰化し, 特に環椎横靭帯は著明に骨化し延髄脊髄移行部を圧迫していた.環椎の前方脱臼を認めるものの, 前方固定後のため軸椎, 後頭骨との不安定性はなかった.後頭下減圧及び環椎後弓切除術施行し, 症状の改善を認めた.本例では環軸椎脱臼があることから, 以前の頚損時に環椎横靭帯が損傷し, その修復過程で骨化が進行したと考えた.環軸椎脱臼の固定後に歯突起周囲の靭帯に影響を及ぼすこともあり長期のフォローアップが必要である.
