EFFECTS OF LONGITUDINAL MATERNAL GLUCOSE CONTROL ON INFANTS OF DIABETIC MOTHERS

厳格な血糖コントロールが母児双方の周産期死亡率を低下させることは周知の事実である. われわれの施設でも1980年から空腹時血糖値100mg/dl以下, 食後2時間値120mg/dl以下を目標に血糖コントロールを行つてきた. そこで今回, インスリンを要する糖尿病妊婦45名を対象に母体の血糖コントロール状態と児の出生体重, 新生児合併症との関係についてretrospectiveな検討を加えた. 良好なコントロールが妊娠32週までに得られた症例には heavy-for-datesの発生頻度が有意に低いことが判明した. そこで妊娠32週を境にコントロール状態を観察し, 32週前後とも良好な群 (G-G群), 32週以降で良好となつた群 (P-G群), いずれも不良であつた群 (P-P群) の3群に分類し3群間で検討した. その結果, heavy-for-datesの頻度は G-G群で有意に低いものの P-G群と P-P群とに差を認めなかつた. light-for-datesの頻度は3群間に差がなかった. 新生児合併症として低血糖, 高ビリルビン血症にも有意差がなかつた. しかし早産, 奇形児はP-P群に有意に高率に発生した. heavy-for-dates発生予防に関しては妊娠32週までの良好な血糖コントロールが重要であるが light-for-datesに関しては血糖コントロールより母体の血管合併症のほうが重要であろう. またlong-termな血糖コントロール状態と新生児合併症との間に直接的な相関は認められなかつた.
