<原著>超短波刺激における小児習慣性扁桃炎のリンパ球サブセットの変化

The provocation test by ultra-shortwave stimulation is wide-spread as the diagnosis technique of tonsillar diseases, mainly of the focal infection accompanied by secondary diseases. However, the focal infection is found in a few of the cases with tonsillopathy and is seldom easier to diagnose than the secondary diseases. In the present study, child habitual tonsillitis, which is found in most of the patients with tonsillopathy and is hard to indicate surgery, was examined by ultra-shortwave mainly on the basis of lymphocyte subsets unlike the traditional studies. Child focal infection, child hypertrophy, and adult habitual tonsillitis were used as the controls. Changes in peripheral CD4 (helper/inducer T cell), CD8 (cytotoxic/suppressor T cell), and HLA-DR (B cell, active T cell) in child habitual tonsillitis by ultrashortwave stimulation were compared with those in the control diseases. CD4 level in child habitual tonsillitis did not differ from those in child focal infection or adult habitual tonsillitis but was significantly enhanced in comparison with that in child hypertrophy. CD8 level in child habitual tonsillitis also did not differ from that in child focal infection but was significantly higher than those in adult habitual tonsillitis and in child hypertrophy. HLA-DR levels did not differ among those diseases. From the above, it may be concluded that changes of the CD4 and CD8 levels in child habitual tonsillitis, unlike those in child hypertrophy, show almost the same response as those in child focal infection and adult habitual tonsillitis and that lymphocyte subsets are useful for the diagnosis of pathologic tonsillar abnormality. Further ultra-shortwave stimulation changed the ratio of CD4 and the distribution of CD4 and CD8 in the tissues of the tonsil. These changes indicated the activation of CD4 and of CD8 in the tissues, which proved the activity of CD4 and CD8 to be enhanced in the peripheral blood and tissues. Thus it may be concluded that helper T and suppressor T levels included in ultra-shortwave stimulation provocation test take a role in understanding the exact pathology of child habitual tonsillitis.
