Twenty-nine patients with total anomalous pulmonary venous return (TAPVR), of ages from one day to 12 years, underwent corrective surgery There were 11 early and three late deaths after surgery. Eight of the 11 patients less than three months of age died. There was no death in the nine patients over one year of age at operation. Eighteen patients underwent cardiac catheterization preoperatively. The left ventricular end-diastolic volume was less than 65% of normal in 4 of the 18 patients and three of them died of low cardiac output syndrome. Left ventricular ejection fraction was less than 0.55 in 6 of the 18 patients and 4 of the 6 died. In those with an unusually small left ventricle, delayed ligation of the anomalous vertical vein was considered preferable. Left atrial and ventricular volumes were gradually restored to the normal after surgical repair. In the neonate and small infant, the late development of anastomotic strictures is a hazard of the operation ; therefore repeated cardiac catheterization after the repair should be done to detect recurrent pulmonary venous obstruction.
