The localization of immunoglobulin (IgG) and complement (C3) in the glomerular tufts and crescents was examined in 5 patients with crescentic glomerulonephritis by light microscopy, immunofluorescence method and electron immunoperoxidase method. Furthermore, the distribution of monocytes and polymorphonuclear cells within the glomeruli in various renal diseases (n=25) was investigated by histochemical analysis (diaminobenzidine staining of peroxisomes) with a light and electron microscope. In crescentic glomerulonephritis, IgG was present in the mesangial, subendothelial and subepithelial areas and electron dense deposits. IgG was not, however, detected in some of the electron dense deposits. Moreover, IgG existed on the surface and within the cytoplasm of crescent formation cells. Peroxisomes were recognized in the crescent formation cells in the same pattern as the proximal tubular peroxisomes. Monocytes were distributed dominantly in the glomeruli of all crescentic glomerulonephritis cases except one and in the other types of glomerulonephritis treated by steroid therapy. On the other hand, polymorphonuclear cells were distributed dominantly in various renal diseases. IgG was not observed on the crescent formation cells by the immunofluorescence method but was identified by the electron immunoperoxidase method. This suggested the violent turnover of IgG in the crescent formation cells. Increased monocytes in glomeruli may characterize crescentic glomerulonephritis, but we do not consider that crescent formation cells are composed of monocytes only. It is probable that crescent formation occurs from proliferation of Bowman's capsular epithelial or proximal tubular epithelial cells as a result of alteration of the function of glomerular filtration.
