<原著>交通外傷による解離性大動脈瘤の一剖検例

Recently the authors have observed a case of dessecting aortic aneurysm, who suddenly died from the rupture of a large dissecting aortic anerysm of the thoracic and abdominal aorta due to contusion of the thorax about seven days after injured in a traffic accident. In this paper the case is reported with autopsy findings. The case was 50-year-old man. When he was injured in a traffic accident (contusion of the anterior thorax), the intima of the superior end of the aorta thoracica was fagmentarily lacerated by the strong shock or stress on the heart and lungs. About four days after the accident arterial blood flowed out of the lacerated site, penetrating the media and dissecting the aventitia to form a large aortic aneurysm (about 40 cm in length and 5 cm in outside diameter). Then three days later the aventitia broke about the aortic arch in the upper end of the aortic aneuryrsm, which induced severe haemorrhage in the left thoracic cavity. Thus it is deducible from the autopsy findings that the sudden death about seven days after the accident was caused by the haemorrhage.
