The study of differentiation and apoptosis as well as proliferation of Paget cells has not been reported yet, although it may be useful to know the nature and prognosis of Paget's disease. Immunohistochemical study of extramammary Paget's disease in 11 patients was done using antibodies to Le^Y and CA 15-3. Le^Y is a carbohydrate antigen belonging to Lewis type antigens and recently known as one of apoptosis-related antigens. In the early stage in which Paget cells were present only in the basal and lower squamous layers, the keratinocytes overlying Paget cells were Le^Y positive while Paget cells were Le^Y negative. Small numbers of Paget cells were PCNA positive. With the stage advanced, Paget cells infiltrated the whole epidermis and Paget cells as well as the reminder of keratinocytes became Le^Y positive. PCNA positive Paget cells increased to some extent, but as the Le^Y positive Paget cells increased, PCNA positive Paget cells decreased. All Paget cells in any stage of Paget's disease were CA 15-3 positive. It is suggested that Le^Y positive keratinocytes in the early stage of Paget's disease may result from acceleration of keratinization and that Le^Y positive Paget cells in the more advanced stages may result from differentiations from multipotentical germinative cells to sweat gland cells rather than apoptosis, although some Paget cells moving into the horny layer may be apoptotic.
