Sweat Allergy and Atopic Dermatitis

We have previously demonstrated that more than half of atopic dermatitis (AD) patients show skin reactions with intradermal injection of autologous sweat. However, several issues remain to be elucidated to further understand the mechanism. We investigated the type of AD showing positive skin reactions to the diluted autologous sweat, and the relationship between sweat allergy (SA) and metal allergy among AD patients. The two types of AD have been proposed. The extrinsic type shows high IgE levels presumably as a consequence of skin barrier damage and feasible allergen permeation, whereas the intrinsic type exhibits normal IgE levels and is not mediated by allergen-specific IgE. We evaluated the prevalence rate of SA and metal allergy using skin patch test with nickel, chromium, and cobalt in 46 extrinsic and 13 intrinsic AD patients. No significant difference was seen in the SA ratio between the two types. On the other hand, a significant increase (100% versus 16.7%) of the prevalence rate of metal allergy was observed in the intrinsic AD patients with SA. Furthermore, the nickel concentration in the sweat of the intrinsic AD patients was higher than that of normal control or the extrinsic AD patients. The chromium concentration in the sweat both of the intrinsic and extrinsic AD patients was higher than that of normal control. These data suggest that the SA is an important factor in aggravation of AD, and metal is deeply involved in the pathogenesis of mainly intrinsic AD and partially extrinsic AD showing allergic reactions to autologous sweat.Skin Research, Suppl. 16: 45-49, 2011
