A case of the diffuse sclerosing variant of papillary thyroid carcinoma:—Cytological and histological features—

Background : The diffuse sclerosing variant of papillary thyroid carcinoma (DSPC) is rare and only a few articles have focused on its cytological appearances. In this study the cytological features of a case diagnosed by fine needle aspiration cytology (FNAC) along with imprint (I), thin-layer (T) and histological features were investigated.Case : In a 35-year-old woman, an ill-defined mass was revealed in her neck with echography. The FNAC findings produced a suspected diagnosis of DSPC and tumor resection was performed. I and T specimens were obtained from fresh cut-surface of the tumor. Cytological features : Numerous solid or spherical clusters with psammoma bodies were scattered in a lymphocyte-rich background. Tumor cells showed conspicuous and slightly eosinophilic cytoplasm with nuclei demonstrating ground-glass appearances and nuclear grooves. Some cells showed oncocytic changes and squamous metaplasia. Squamous metaplasia was easily observed in I and detailed structure of papillary clusters with psammoma bodies were clearly observed in T. Macroscopic findings : A diffuse whitish lesion was found in the left and lower part of the right lobe with a tumor of about 1 cm present in the left lobe. Histological features : In addition to ordinary papillary carcinoma, lymph vessel invasion was prominent. Stromal fibrosis, lymphocytic infiltrates, psammoma bodies and squamous metaplasia were also observed.Conclusion : The FNAC specimen revealed characteristic features of DSPC and was useful for cytological diagnosis, in addition I and T specimens were also useful for confirmation of the tumor's precise cytological features.
