An analysis of the Bethesda system for reporting thyroid cytopathology and a proposal for recommended clinical management

Objective : The Bethesda system for reporting thyroid cytopathology was recently proposed, in which the implied risk of malignancy and recommended clinical management was included.Study Design : To verify whether the description is applicable to Japanese cases or not, we examined 183 "suspicious for malignancy (SFM)" cases in our hospital.Results : SFM was 1.7% of all aspirated thyroid cases, and the positive predictive value in the resected cases was 95.8%. Papillary carcinoma and malignant lymphoma were suspected in 88.0% and 10.4% of the SFM cases, respectively. 64.5% of the SFM cases underwent surgery. Non-resected cases mainly consisted of follow-up for microcarcinoma. Three false positive cases were adenomatous nodules with prominent papillary proliferation.Conclusion : According to our data, the SFM cases should undergo surgery except for microcarcinomas without metastasis or invasion of the surrounding tissue.
