We report a case of parotid gland lymph node Merkel cell carcinoma metastasis arising in facial skin and diagnosed by aspiration cytology. Right parotid lymph node metastasis clinically suspected from Merkel cell carcinoma was supplemented by aspiration cytology. The specimen contained numerous carcinoma cells with small round nuclei, finely stippled chromatin, scarce cytoplasm, and dot-like Merkel cell carcinoma-specific CK20-positive intracytoplasmic inclusions. Such metastasis is only confirmable by aspiration cytology with immunocytochemistry without taking a histological specimen.
