Ultrastructure of the healing process of perforated round window membrane in the guinea pig.

The round window membrane of the guinea pig was surgically perforated and its healing process was studied by light and electron misroscopy. A slit perforation, 0.5-0.8mm in length, going along the fibers of the membrane was made with a microknife. [Results] 1) Within ten days, all the perforations closed. 2) In the first stage, moderate hypertrophy of the outer and middle layers was observed around the perforation, about 0.2mm in width, which looked like a head of a snake in section. Some hypertrophic cells of the outer layer protruded into the middle ear cavity. The inner layer peeled off from the middle layer and its free ends floated in the perilymph. 3) In the second stage, the both edges of the slits closed and the middle layer healed up. As a result, it looked like a shallow crater in SEM view. Inside the crater, the epithelial cells of the outer layer were growing on the middle layer. 4) In the last stage, the hypertrophy of the outer and middle layers was decreasing with the lapse of time. Recovery of the inner layer was the latest, but the new inner layer growing on the middle layer was observed.
