Hearing Results of Type IV Tympanoplasty with Ossicular Reconstruction

Forty six cases of type IV tympanoplasty with ossicular reconstruction are presented.<BR>Hearing was analyzed according to types of the diseases and types of reconstruction employed. The hearing result was classified as successful when the postoperative hearing level met at least one of these three conditions: 1) hearing level above 40 dB, 2) hearing gain more than 15 dB or 3) air-bone gap less than 20 dB.<BR>Success was obtained in 27 (59%) of the all patients. It was found that primary operations for aural cholesteatoma and for middle ear anomaly produced successful hearing results (86% and 75% of the cases, respectively). On the other hand, in the cases of revision operation, adhesive otitis media and chronic otitis media, the incidence of success was not very high (51% on the average). There was no significaut difference between the types of ossicular reconstruction, but in a total of 24 patients who recieved a ceramic ossicular replacement prosthesis (CORP), there were five patients who showed extrusion of the CORP (21% of the cases), and unsatisfactory hearing gain.
