Dislocation after total hip arthroplasty, whether develops early or late, can evolve into a devastating complication when it becomes recurrent.A number of factors related to surgery contribute to dislocation, e. g, malposition of the prosthesis, nonunion of the great trochanter, surgical soft tissue injury, selection of an inappropriate prosthesis, and impingement on the bone, soft tissue and the prosthesis itself. Because the etiology is multifactorial, it is often difficult to establish the cause of dislocation with precision and confidence. This makes it difficult to predict the outcome of treatment except when the prosthesis is poorly positioned, a situation that can be identified radiographically.As salvage management for severe instability of the hip in patients in whom the standard reconstructive procedures have failed, we are occasionally compelled to choose between capitative or bipolar articulations instead of total hip prosthesis.To minimize the incidence of unfortunate complication, each surgical risk should be identified and its risk minimized.We will review and discuss our method to prevent hip dislocation following total hip arthroplasty.
