Pulmonary embolism and venous thrombosis during pregnancy has become an important cause of maternal mortality in Japan as well as in Western Europe and North America where hereditary thrombotic abnormality is relatively common. We report a fatal case of pulmonary thromboembolism with a sudden onset on the first postoperative day of Cesarean delivery without any risk factors such as advanced maternal age, obesity or preeclampsia other than the operation itself. At autopsy, fresh and organized thrombosis and thromboembolism were diffusely found throughout the peripheral branches of pulmonary arteries confirming the diagnosis of pulmonary embolism. Most of pulmonary embolism cases are considered to be closely related to deep venous thrombosis, both of which have unspecific symptoms. Prophylaxis for every patient with Cesarean delivery may be necessary considering the absence of risk factors in current case and the disease is often critical once symptoms become evident.
